In any health facility or organization environment, an ethical dilemma can come up when challenges arise and a decision needs to be made taking into consideration moral principles (Rainer, Schneider & Lorenz, 2018). Therefore, the result of the decision starts with complex clinical affiliations. Nurses usually undergo a course of decision-making that upholds resilience to do away with unwanted results. In nursing, ethics denotes being vigilant with the principles of an occupation connected with many approaches and principles of ethics. That means showing compassion and kindness, known as benevolence, subsequent, by being an accountable and committed healthcare worker labeled as the non-maleficent (Hamric et al., 2014). As being the tireless advocate of the ailing individual, a nurse can acknowledge their privilege to personal willpower, which is known as autonomy. Finally, there must be a principle of fairness that needs balance in each healthcare verdict and equal treatment for all ailing individuals. This ideology is vital, which helps enhance the quality of care and regulates all nurses to guide the hardships that commonly occur in the nursing occupation.
The most usual circumstance that occurs in the health facilities is the ethical dilemma which encompasses professional matters, and this is what is referred to as individual autonomy. This is where this nursing principle turns out to be compared to goodwill and fairness (Hamric et al., 2014). One situation that I have experienced as a nurse has been when the doctor’s orders do not align with the patient’s wishes. As a nurse, I need to understand my position as a clinical professional, but also as an advocate for my patient. I would try first to allow my patient to verbalize his/her concerns regarding the plan of treatment and then I would provide the appropriate education regarding the matter so they could take an informed decision.
Question2
The nursing code of ethics documents ethical standards such as autonomy, non-maleficence, and respect that should shape care provision. Autonomy articulates patients’ independence in decision-making that care providers need to respect. Non-maleficence articulates for less harm to patients (Haddad, L. M., & Geiger, 2020). I reflected on these values, and I could not decide or overlook the patient’s decision since it would undermine autonomy and non-maleficence. For this reason, I proposed in-home care for the patient that would involve a visiting nurse at least twice per week and the family to employ an assistant to help their father with house chores, management of diabetes and compliance with medication requirements. They explained to their father about the second option, and he embraced it. The case portrays challenges that nurses, and other care providers encounter in care provision. It also warns care providers from sacrificing ethics and patients’ interests in demanding situations. Patients should always come first.
Question 3
Advocacy is the term that is used to describe the support of a particular idea or cause. This concept is crucial as it allows one’s voice to be heard
on issues and decisions that are important to them, as well as how these decisions can impact their life. Nurses advocate on all realms including for
their patients, patients’ families, as well as for themselves and colleagues. This advocacy can consist of attempts to provide and promote better health
for their patients, or even improving healthcare in general. One way nurses can improve healthcare through legislation at either the state or federal
level is by advocating for policy change. Health policy affects every nurse’s daily practice, that’s why it is essential that nurses be represented in new
policies or policy reform (Mason et al., 2016). Nurses advocating for good causes in policies play a significant role in improving health, as nurses are
the ones who implement and execute the duties that are described in the healthcare policies. Furthermore, nurses have first hand knowledge of the
demands of healthcare, situations or conflicts, and what is needed or required for advancement. This kind of “first hand” intelligence is essential to
properly inform policymakers when making critical decisions about health system reform, regulatory changes, care coordination, and health
information technology (Anders, 2020).
In addition, nurses being a part of and advocating for the creation of hospital committees and councils can play a factor in policy change within
the organization. These local committees work collaboratively with hospital administration to make decisions for the hospital. Committees range from
policy and procedure committees, to infection control prevention, and control committees to quality initiatives (Anders, 2020). For example, my
hospital has the Unit Council Committee, which is made up of a staff nurse from every unit of the hospital. An issue that was addressed by this
council was related to insulin errors. Therefore, they created a policy to reduce the number of errors and improve patient outcomes by requiring dual
verification for insulin administration.
Question 4
Nurses and other healthcare providers can impact public policy by using their advocacy skills. In particular, the nurses can use their clinical experiences to demonstrate the needs of the patients, families, communities, and populations to advance public policy on quality health care services (Lewinski & Simmons, 2018). The nurses can influence the legislation of policies that will lead to accessibility to appropriate, equal, effective, and efficient care for the populations, patients, and communities. Therefore, advocacy is an essential element in policy development in state and federal governments. Advocacy involves persuading individuals to consider the views of an individual to promote a certain topic in society. As a result, the discussion is based on different actions that a nurse can use to contribute to state or federal policy development as an advocate.
In state or federal legislation policy development, I will like to provide information to elected officials to influence them in making policies and decisions to influence health care. While working in the healthcare environment, I have the opportunity to provide the elected officials with accurate and current information that will be critical in the policy development process (Lewinski & Simmons, 2018). In this case, it is important to provide information that is based on credible research, evidence, facts, and understandable language. The provision of such information should also be done cooperatively to show goodwill (Hays & Beverly, 2021). For example, if I provide information on healthy lifestyle choices for children in school to the state legislature to influence their decisions on the school meals, the information will be based on credible, valid, and reliable research, facts, and evidence to increase the chances of acceptance.
Apart from providing information to elected officials to influence their decision-making and policy development process, I would like to also introduce a new bill to Congress or state legislation to influence change in the healthcare system (Abbasinia et al., 2020). In this case, I will need to find an elected official with who I will collaborate to introduce the bill to the state or federal legislators. In particular, I will be meeting up with the legislator or any representative of the legislator to craft and draft the bill that can be used to support quality, equal, effective and efficient health care. I will also be involved in finding co-sponsors and supports that can help in influencing the position of other legislators on the issue. I know getting the attention of other legislators, I will need massive support which persuades the rest. Therefore, I will focus on developing coalitions with different individuals to gain support for the new bill to advance the availability and accessibility of health care for the whole population.
